Current Service Provision
- HIV Testing and Treatment: US Government partners, including a USAID implementing partner (IP), are providing HIV testing and treatment services. The stop-work order for USAID IP IntraHealth, responsible for key population interventions, has been reversed.
- Service Continuity: All facilities/service points providing antiretroviral therapy (ART) are operating at full capacity. Data collection, quality control, and collation continue without interruption.
- Supplies: Stocks of antiretroviral (ARV) supplies, HIV viral load (VL) test kits, and condoms are available with minimal distribution disruptions. Condom stock levels are sufficient for the next 12 months, though supply chain aspects have been impacted.
Immediate Risks and Disruptions
- Prevention Services: There are significant disruptions in HIV prevention services for key populations, including access to pre-exposure prophylaxis (PrEP), HIV counseling and testing, and ART services for sex workers, men who have sex with men, transgender persons, people who use drugs, and prisoners. The country also reports inability to provide opioid agonist therapy and other harm reduction services for people who use drugs and decreased availability of HIV prevention services in prisons.
- Human Rights and Legal Environment: Programmes addressing stigma, discrimination, and enabling legal environments have stopped. There is no data collection on human rights issues, including increased stigma and discrimination.
- Government Response: The government has not formalized a task team to discuss service delivery impacts and mitigation measures. Internal meetings with US partners, mostly funded by CDC, are ongoing.
Politically Relevant Updates
- Funding and Support: US funding for anti-stigma and anti-discrimination work has ceased without redirection to other programmes. The government is not covering the salaries of PEPFAR-funded human resources.
- Civil Society and UN Response: Civil society organizations (CSOs) remain resilient and adaptable. The UN Country Team (UNCT) is engaged in mitigating the impact of the US funding cuts.
